How to Request a Meal Plan Accommodation

  1. Schedule a meeting with Registered Dietitian, Meghan Dempsey, to discuss your specific needs and how Catamount Dining may be able to accommodate you.
    1. To schedule a meeting, please use our online scheduler.
  2. Fill out a release of information form to allow Health Services to collect any pertinent medical documentation related to your request. Medical records must demonstrate that you have a diagnosis or have received treatment with a licensed healthcare provider for a medical condition that impacts your ability to fulfill your nutritional needs in Catamount Dining facilities. If your healthcare provider wishes to summarize your condition in a letter, please ensure the letter includes adequate information such as your symptoms or diagnosis and treatment plan.
    1. You may access this form online under the “Forms” tab.
  3. Contact Health Services at 828.227.7640 to schedule an appointment to review your medical documentation.
    Please note that you will not be able to schedule an appointment with Health Services to review your medical information until you have spoken with the Registered Dietitian.
  4. Health Services will provide you with a note evaluating the medical severity of any food-related medical condition (ex: food allergies, Celiac Disease, food sensitivity, etc.) that you may forward on to the Dietitian.

    Please note that Health Services does not provide any recommendation for the approval or denial of any request and serves only to evaluate the medical severity of your condition.

  5. After reviewing all pertinent information, a decision will be made regarding your decision, and you will be notified via email.
    1. Expect to receive a final decision within 1-2 weeks of delivery of all necessary information.
